Assumption of Risk/Medical Emergencies: I assume all risks associated with participation at Foursis Dance Academy, LLC or Foursis Academy of Dance, LLC, including but not limited to falls, contact with other participants, and other reasonable risks and conditions associated with the dance/tumbling classes. All such risks to my child are known and understood by me. I authorize Foursis Dance Academy, LLC and Foursis Academy of Dance, LLC and it's staff to locate qualified and licensed medical personnel and/or transport said student to an appropriate medical facility in the event that it may become necessary. I allow Foursis Dance Academy, LLC and Foursis Academy of Dance, LLC and staff to authorize for emergency treatment for an illness or injury to my child if qualified medical personnel consider treatment necessary and to perform the treatment.

Release of Liability: As a parent or legal guardian of the aforementioned student, I authorize my son or daughter to participate in the dance/gymnastics classes at Foursis Dance Academy, LLC. (FDA) and Foursis Academy of Dance, LLC (FAD) I understand I will be notified as soon as possible in the event of an emergency. I or my insurance company will assume all expenses of such treatment. Foursis Dance Academy, LLC and Foursis Academy of Dance, LLC, including all staff shall not be held responsible for any injury incurred as a result of my son's/daughter's participation in Foursis Dance Academy, LLC or Foursis Academy of Dance, LLC.

PAYMENT POLICIES

Auto-pay: Tuition must be paid by automatic bank account, debit or automatic credit card payment. Tuition will be debited the first of every month. The month of September (or your first month) is non-refundable. A registration fee of $25.00 per dancer will be charged with September (or your first month) tuition, and is non-refundable. All charges will appear as being from Foursis Dance Academy, LLC or Foursis Academy of Dance, LLC.

Non-Sufficient Funds: I hereby authorize the studio to electronically debit my bank account for the amount of any NSF authorization and a $25 authorization fee for any payment not cleared within 3 days of the first of the month.

Withdrawal and Refunds: There is a 2 month minimum for all classes. To withdraw from any class a parent or guardian must inform Foursis in person and complete and sign a withdrawal form. This form can be completed at the front desk. If a member withdraws the 1st-15th of the month your tuition will not be charged for the following month. If a member withdraws on the 16th-31st of the month you will still be charged on the 1st of the following month. Foursis reserves the right to terminate classes to any student without notice. In such case a refund for unused classes will be given. FDA and FAD reserves the right to have students sit out of class if fees are not paid and up to date.

PARENT / GUARDIAN INFORMATION

Communication Policy: Newsletters are emailed out monthly. It is your responsibility to check and make sure you have received any and all paperwork/emails. It is the responsibility of the parent or to be aware of all academy activities, such as recitals, extra classes, and dates the studio is open or closed. It is the responsibility of the parents or to inform Foursis of any address, telephone, billing or email address change and to update in your customer portal.

Care of Students: Foursis is not responsible for providing care before or after any class. Students are not to be left at the studio for excessive time before or after class. Students should not arrive earlier than 15 minutes prior to their class and must be picked up within 5 minutes after their class has ended. For the safety and flow of students and traffic outside, we ask that you follow our parking and entrance procedures. For the safety of everyone, no parents or siblings should enter any of our dance or gymnastics studios/gyms whether they are occupied or not.

Covid 19: In the event of a natural disaster, global pandemic or any other unforeseen circumstance out of our control, Foursis reserves the right to provide alternate means of instruction.

Privacy Policy: Foursis is committed to online security. You can rest assure your payment and personal information will not be shared with anyone. Your personal information is for Foursis’s use only.

Canceling Classes: Foursis will be closed and have no classes when the Waterford Graded School District (JT1) closes or Waterford Union High School cancels afterschool activities due to inclement weather for FDA and when the New Berlin School District or New Berlin High Schools cancel for FAD. It is the judgment of the owners if the studio will close due to bad weather on a Saturday. If you are unsure as to whether there is class please call 262-514-4240 or visit our social media. Our School Year Session consists of 32 weeks of classes over an 9 month period and tuition is conveniently debited monthly. We have also built in 2 weeks of classes in our session for circumstances out of our control, such as weather or power outages. If the 2 weeks of classes are not used prior to the end of the School Year Session those classes run as scheduled with no additional charge. If closures for a specific day or class exceeds the 2 days that are built into our schedule, make-up classes are scheduled. Foursis will be closed during most major holidays. A calendar will be given in advance.

Dance Recital Costumes: Dancers who enroll in our full 9-month School Year Session (Sept-May) will receive a FREE recital costume for each class in which they are enrolled in. Dancers who enroll after September 30th will incur a $75 costume fee per class. The costume fee will be debited the day you register for any dancer who enrolls after September 30th. All required recital tights and shoes must be purchased separately through Foursis.

Gymnastics/Tumbling Recital Leotards: Recital Leotards will be debited February 1st for every student enrolled in gymnastics or tumbling in the current School Year Session.

Cancelled Classes: Classes with less than four students may be cancelled, combined, or rescheduled at Foursis’s discretion.

Substitutes: Foursis reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and cannot arrange a substitute, the missed classes will be made up.

Photo Release: Foursis is hereby granted permission to take photos of the students to use in brochures, studio web site, posters, advertisements, or other promotional material. Permission is also hereby granted for the studio to copyright such photos in our name.

Injuries: Parents/guardians of minor students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal dance or gymnastics activity or any other activity conducted by the students before, during or after class time.

ACADEMY POLICIES

• Everyone must wait in the studio lobby until your class begins. Students will be called into your studio, lesson or gymnastics area by your instructor. No one is allowed in any of these areas unless an instructor is present.

• No food or drink allowed anywhere except the studio lobby or student lounge.

• No gum chewing is allowed in class.

• For the safety of every student, Foursis reserves the right to have a student sit out of his/her class if they do not meet Dress Code Guidelines.

• Do not leave siblings/friends not involved in dance class at the studio to wait for siblings and/or friends that are in class.

• For the safety of our students there is no parent watching for our dance, gymnastics or tumbling classes.

• Recital/Showcase costumes/leotards will not be handed out to any student unless their class fees are paid through May. Any student who chooses not to complete the School Year Session will not receive a costume. Recital Leotards are non-refundable.

• If you choose to drop a class/lesson, no refunds are given. This includes costume/leotard charges.

• Any Foursis apparel is custom made and once ordered may not be returned or refunded. All leotards and dance/gymnastics accessories may only by exchanged if unused with tags attached and in original packaging within one week of purchase.
